4/16/2023 EnchantaVaughn Approved 1. Behavior: Upon arrival to the home setting, QMHP-C was greeted by Ms. Hodge and Ashiyah. Ms. Hodge was engaged in a discussion to determine Ashiyahâs behaviors and any updates on her behavior. Ms. Hodge discussed with QMHP-C Ashiyah has been in verbal altercations with her siblings. 2. Behavior: QMHP-C entered Ashiyahâs home to conduct individual session. Ashiyah appeared to be in a pleasant mood evident by positive facial expressions and communication. QMHP-C focused on anger and communication skills due to Ashiyahâs current aggression in the home. 1. Intervention: QMHP-C recapped anger management and encouraged Ashiyah to share how she has been utilizing coping skills in her daily interactions with her family. QMHP-C discussed with Ashiyah how anger and negative behavior impacts her interactions with those around her. QMHP-C discussed with Ashiyah compliance with her mother rules. 2. Intervention: QMHP-C encouraged Ashiyah to put herself in her motherâs position and consider the responsibilities and how it makes her feel when she is disrespected by her for the Ashiyah and her current standings with those rules and regulations. QMHP-C discussed with Ashiyah how her mother will continue to be firm with her rules until Ashiyah demonstrates the ability and willingness to consistently comply with those rules in place. QMHP-C discussed with Ashiyah different strategies she can use to help her home life be less frustrating and stressful. 1. Response: Ashiyah actively engaged in conversation with QMHP-C during session. Ms. Hodge reported that the Ashiyah became aggressive with her verbally for the first time she can remember. She reported that the Ashiyah used profanity toward her. Ashiyah expressed to QMHP-C that she gets tired of arguing with her mother so now she goes to her room. 2. Response: During the session, Ashiyah shared that she has begun writing short stories about different events that have taken place in her life and that helps her stay out of trouble by avoiding arguments with her mother. Ashiyah reported that she journals to avoid expressing herself to others which she has always struggled with doing. Ashiyah admits that she still does ignore her mother at times, but she eventually does what she asks her to do.
